Police Clearance Letter or Name-Based, Statewide, Conviction-ONLY Background Check
This type of background check is needed for a police clearance letter for immigration or legal purposes, obtaining a Visa in another country, and or if you would like to obtain your own conviction record.
Fingerprint-Based, Statewide, Conviction-Only Background Check also known as UCIAF Background Check
This type of background check is needed for immigration, legal, or school/certification purposes when a fingerprint check is required.
Please note: The Berwyn Police Department will no longer provide a letter for name-based background checks; those results must be obtained by the Illinois State Police. All Berwyn incident court dispositions are provided by the Cook County Clerk’s Office; the nearest location is at the 4th Municipal District Maywood Courthouse, 1500 Maybrook Drive, in Maywood.
The above mentioned background checks are performed by the Illinois State Police. These are the following ways to obtain the background check you need:
- Police Clearance Letter or Name-Based, Statewide, Conviction-ONLY Background Check:

- paper application (soon to be phased out)- email BOI.Customer.Support@illinois.gov and request for a UCIA Name Inquiry (ISP 6-405B) application and the fee will be collected by check or money order
- Fingerprint-Based, Statewide, Conviction-Only Background Check:
- conducted in-person at an Illinois-approved Live Scan vendor
- list of vendors
- fee collected at vendor
Please note: None of the background checks mentioned above will produce national (FBI) results. Those background checks need to be requested directly from the FBI Criminal Justice Information Services (CJIS) at FBI
Illinois Arrest and Conviction Record Requests
If you are looking for a state-wide criminal history transcript that includes Illinois arrests and convictions for the purpose of correcting, expunging, or sealing a record and or for identity theft purposes, you need to follow the information below.
This type of record request is performed at the Berwyn Police Department and is called Access and Review. You must provide a current form of ID and pay $50 (cash or money order). A booking officer will process a digital “fingerprint card” that is automatically sent to the Illinois State Police who in turn will mail a criminal history transcript to the address provided by you.
